Personalized Specialty License Plate - Ordering Instructions
Step 1: Visit your local tax collector office to process in person. Cannot be ordered online. Search for local office here
https://www.flhsmv.gov/locations/.
Step 2: To make the process smoother, customers should first go to the FLHSMV website to confirm whether the name they want is available, also fill out the required forms beforehand to bring to the office. See links in step 3 and step 4.
Step 4: Application form to fill out for personalized license plate (HSMV form 83043) here
https://www.flhsmv.gov/pdf/forms/83043.pdf
Step 5: There cannot be duplicate names or number/letter designations on any FL state license plate. Eg, if "Eric" is already taken, then one cannot put "Eric" on a FSWI plate.
Step 6: Once the in-person ordering is complete, the personalized plate is then delivered to the specific tax collector office from which the order was placed. The office will then ship it directly to the customer, or the customer may pick it up from that office.
NOTE: We advise that you do not order personalized plates through third-party websites. They are not operated or approved for processing by any government agency or the FLHSMV.
